MSW Field Profiles 2018-19

MSW Jared Anthony Maciejewski 2018-2019 Agency Name: Wellspan Ephrata Community Hospital Profile: The focus of this agency is providing psychiatric hospitalization for patients in need of acute and extended acute care. This agency serves adults and patients from Adams, Lancaster, Lebanon, and York counties. Field Placement Responsibilities: My role is this setting consists of conducting bio-psycho-socials and incorporating my perspective during daily treatment teammeetings. Encouraging family involvement and providing family therapy when necessary, are also services provided. Ensuring each patient has a safe discharge plan by making appointments, talking with supports, and working with patients to create a safety plan. I've had the opportunity to conduct statistics regarding follow-up, utilization of software, and more. Future Goals: Once earning my MSW, I hope to work with individuals with mental illnesses. I'll be applying for licensure and would be fortunate to find a position with a clinical supervisor. Department of Social Work
