They offer a wide variety of needs to include Med Bridge: short - term care, long - term care, and outpatient rehabilitation. Serving a low - income, at - risk, under - served population, HCR ManorCare intervenes to help bridge the gap. Field Placement Responsibilities: As a social work student, my responsibilities include providing appropriate bio - psycho - social assessments, referral assistance, treatment, rehabilitation and supportive care to the patients and their families.
