MSW Field Profiles 2019-20

MSW Caroline Cooley 2019 Agency Name: St. Joseph's Medical Center Profile: The mission of St. Joseph's Medical Center is to "nurture the healing ministry of the church, supported by education and research. The health care professionals at St. Joe's strive to ensure patients and their families are met with respect and empathy throughout the healing process. Field Placement Responsibilities: As a social work intern in the Care Coordination Dept. I will work with the interdisciplinary team consisting of nurses, therapists, physicians, and dietitians to help patients and their families overcome barriers to discharge. Insurance/ prescription drug coverage, financial instability, lack of social/ emotional support, and family violence are just some of the problems patients face that the LSW's need to address before a safe discharge can occur. I will be networking with professionals and agencies in the Berks County area that can offer services and support to patients and their families in order to help themmeet their biopsychosocial needs upon discharge. Future Goals: My goal is to obtainmy LCSW in order to continue and improve my work in the field of geriatrics so as to enhance the lives of older adults and to better support their families as their loved ones age.
