MSW Field Profiles 2019-20

MSW Carrie Ann Callahan 2019 Agency Name: HCR Manor Care Bethlehem Profile: HCR Manor is a skilled nursing facility that provides 24-hour care to individuals over the age of 18, ranging from rehabilitation to hospice. The social work department will focus on providing assessments, such as Minimal Data Set (MDS) and complete bio-psycho- social assessments on assigned residents. Social Services works to ensure a safe return home for short termpatients. Field Placement Responsibilities: My responsibilities as an intern at HCR ManorCare include completing MDS Assessments, Admissions/ Discharge Assessments and Client Satisfaction Surveys. I also am responsible to complete concern forms regarding patient concerns and comments. I also make sure that the documentation of all exit-seeking patients are correct and up to date. I regular send out Care Conference letters to remind patients of their scheduled date. I also aid in the process of signing up patients who are interested in receiving dentistry, audiology, and eye care at the facility. Future Goals: My future goals are to continue to work with the elderly population. My long term goal is to earnmy DSW and become a professor. Department of Social Work
