MSW Field Profiles 2019-20

Lesley Reyes-Cerda 2019 Agency Name: Lehigh Valley Health Network, Street Medicine Profile: Lehigh Valley Health Network, Street Medicine is a program that provides medical care to homeless individuals and families in Lehigh and Northampton county. The care is provided by physicians and nurses at soup kitchens and shelters. Care is also provided to those under bridges and in the woods. Street Medicine strives to provide services that integrate clients back to society. This programbelieves that everyone matters. Field Placement Responsibilities: My role as the MSW intern is to be an educator to the patients and broker them to services and resources that will empower them to become independent and self-sufficient. My field placement responsibilities include assisting with medical insurance applications, food stamp applications, job applications, government- issued phone applications, making doctor appointments, identification obtainment, social security/disability income applications, etc. In addition, I play the role of an educator by training the new upcoming MSW students. Future Goals: My future goal is to obtain the necessary credentials (LSW,LCSW,DSW) to have my own practice and serve individuals and families who have experienced and are experiencing trauma due to their immigration status.
