Jasmine Jones 2019 Agency Name: Lehigh Valley Health Network, Comprehensive Health Services Profile: Lehigh Valley Health Network (LVHN) Comprehensive Health Services (CHS) serves the population of patients diagnosed with HIV and AIDS. CHS utilizes a multidisciplinary approach to providing services to patients. Patients are provided with case management, mental health, treatment, and prevention services. The case management department focuses on meeting the holistic needs of the patients, eliminating barriers, and promoting the well-being of their patients. Field Placement Responsibilities: At CHS, I am responsible for engaging with patients through the intake process and follow ups. I perform the tasks of completing Special Pharmaceutical Benefits Program applications and renewals, as well as, completing case management recertification. I provide interventions to patients on an ongoing and as needed basis. I also perform the task of documenting and conducting outreach visits. Future Goals: My future goals consist of completing my MSW and getting my LSW and LCSW. My future career goal is to become a medical case manager within the hospital setting. Department of Social Work
