LVH Pocono is part of the Lehigh Valley Health Network which also comprises LVH -Cedar Crest, LVH - Muhlenberg, LVH - Hazelton, and LVH - Schuylkill. Field Placement Responsibilities: Responsibilities at this field placement include conducting assessments of patient and family needs, coordinating discharge plans based on interdisciplinary team recommendations with the goal of ensuring continuity of care. Responsibilities also include facilitating the appropriate use of hospital resources and communication with insurance companies and community partners for post acute services.
