Stacy AnnWisdom 2019 Agency Name: St. Luke's University Health Network Home Health Profile: St Luke's University Health Network Home Health is a non-profit, Medicare-certified home health agency which has provided quality health care in the Lehigh Valley for more than 95 years. We work in the home of patients discharged from area hospitals and other health care facilities, as well as those of patients referred to us by their physician. We provide intermittent skilled nursing and/or therapy and social work services. Field Placement Responsibilities: As a medical social worker, I assess patients social and emotional factors related to illness, care needs and available resources including family support. As a family social worker I integrate family in the environment perspective on a mezzo level in micro settings. Future Goals: After graduation I would like to pursue a career in clinical social work as an autonomous practitioner, collaborating with advocacy groups to provide support for persons with intellectual disabilities.
