APPENDIX I The information below is in regards to completion of volunteer background checks for your Council of Trustees volunteerism. The following documents and instructions below provide the details you will need to complete the required clearances: • Volunteer Form - Please complete and return to me. This can be emailed directly to me at reichertre@kutztown.edu • Pennsylvania State Police Clearance - As a volunteer, there is no charge for this background check, you will need to go to this link: https://epatch.pa.gov/home and follow the directions to complete this online. Please put down Kutztown University as the Volunteer agency, and use the phone number 610-683-1353 when asked. When the clearance is complete, I will need a copy for our files. You may forward that to reichertre@kutztown.edu • Child Abuse History Clearance - Included are instructions for completing the Child Abuse clearance online. However, you will be selecting “Volunteer” as the purpose for clearance (page 12 of the instructions), rather than a School Employee. As a volunteer, this clearance is free once every 57 months. Please email me a .pdf attachment for the Child Abuse History Clearance when completed to reichertre@kutztown.edu. • FBI fingerprint Clearance - Due to the volunteer nature of your position you may not be subject to the FBI fingerprinting check. If you’ve been a resident of Pennsylvania for the entirety of at least the last ten consecutive years prior to the date of your background check you may forgo fingerprinting. Please complete the Volunteer Exemption for FBI BC Check form and return to me at reichertre@kutztown.edu. You will still need to complete both the Pennsylvania State Police Clearance and the Child Abuse History Clearance and return to me as noted above. • If you have not lived in PA for ten years, you will need to do the FBI fingerprint clearance, please let me know ASAP, and I will send you instructions for FBI fingerprinting. • If you have current, (after 2020) clearances for any of these, please send in copies with your volunteer form, as we may be able to use these instead. • All clearances must be completed.
