MSW Rebekah Brossman 2021 Agency Name: Advantage Home Health & Hospice Profile: Advantage is an agency that provides home health, hospice, nursing, and bereavement services to the elderly and their families regarding end - of - life care and counseling. Field Placement Responsibilities: As an intern I will shadow and work closely with agency social workers, assist in psychosocial, socioeconomic, and long - term care planning, make bereavement calls, do psychosocial assessments, attend and lead support groups, and complete documentation for patients who are terminally ill. I will serve as a broker and connect clients to resources within the community. In the role of an enabler I will be display warmth, genuineness, and empathy as I support families and patients during the dying process. I will also attend agency meetings and groups and interact with a multi - disciplinary team whose own ethical and professional backgrounds are diverse from social work. Future Goals: I hope to obtain my MSW, my license, and eventually work in a health care setting. Department of Social Work
