MSW Field Profiles 2021-22

MSW Nancy L Fazio 2021 Agency Name: Manor Care Profile: Manor Care is a skilled nursing facility that provides short term rehabilitation stays for recently hospitalized patients, as well as long term care for patients with severe disabilities. Rehabilitation provided can range from 7 - 14 days of intensive physical therapy, occupational therapy, and speech therapy. Long term care residents range from diagnoses of dementia to severe mental illness. Field Placement Responsibilities: As a social service intern, I am responsible for providing biopsychosocial assessments, MDS assessments, as well as interact with family members regarding their loved ones. I work with the interdisciplinary teams to provide safe and affective interventions and discharges for patients. For long term patients, I provide quality assurance surveys that touch on all aspects of their daily lives in the nursing facility. I work with many different agencies in the area to provide residents with necessary interventions regarding their care. I also order medical equipment and transpiration for patients. Future Goals: My future goals are to gain more experience in medical social work at a hospital with Hematology/Oncology patients. From there I am interested in providing individual therapy through my own private practice. Department of Social Work
