MSW Morgan Harper 2021 Agency Name: Northeast Family Health Care Profile: Northeast Family Health Care located in Quakertown is an outpatient Medication Assisted Treatment facility. Northeast Family Health Care offers medication management through methadone, suboxone, and vivitrol. Rooted in a patient centered, and harm reduction approach with the ultimate goal to assist each patient in achieving their recovery goals and living a fulfilling life. Field Placement Responsibilities: While placed at Northeast Family Health Care I will be assisting patients in developing individualized treatment plans catered to their strengths, needs, and abilities. Following individual treatment plans I will assist patients through a number of evidence - based practices and various techniques such as; Individual counseling, family and group counseling, Cognitive Behavioral Therapy, and Motivational Interviewing. I will assist patients in maintaining medical stability while remaining engaged in treatment. Above all else, I will work with patients to support them through their recovery journey. Future Goals: My future goals are to remain in the substance abuse field, and work towards becoming a Certified Alcohol and Drug Counselor. Department of Social Work
