MSW Field Profiles 2021-22

MSW Jasmine Foster 2021 Agency Name: WellSpan Philhaven Profile: The focus of Philhaven is to provide a continuum of mental health services. Philhaven offers over 20 programs including, but not limited to inpatient hospitalization, adult and child day programs, addictions services, peer support and more. Field Placement Responsibilities: In the role of a therapist I will provide individual, group, and family therapy. Collaborate with clients to develop initial treatment plans. I will provide service towards treatment plan goals using different evidence based treatment to include, but not limited to Cognitive Behavioral Therapy (CBT) and Dialectical Behavioral Therapy (DBT). I will attend weekly collaborative teammeetings with other therapist on the team and the program Psychiatrist to seek supervision and guidance to best treat each client. I will document current, accurate observations of the client in EPIC. I will assess clients progress and modify or extend treatment in the program accordingly. I will also coordinate aftercare based on the clients ’ needs to transition client from this level of care. Future Goals: My future goal is to become licensed, working towards a LCSW and working with military members, with a focus on trauma and PTSD. Department of Social Work
