MSW Field Profiles 2021-22

MSW Riley Kampia 2021 Agency Name: WellSpan Ephrata Community Hospital Profile: WellSpan Ephrata Community Hospital ’ s mission is to work as one to improve health through exceptional care for all, lifelong wellness, and healthy communities. Specifically, within the Case Management Department, social workers assist with all discharge planning needs that a patient may have. Field Placement Responsibilities: Within the Case Management Department at WellSpan Ephrata Community Hospital, the social workers and I work as a team to meet with every patient within the hospital. Through meeting with each patient we are able to assess their needs, provide any needed outside resources, and help the patients through the process of receiving continued care at their next facility or within their home once discharged. Other tasks that the social workers and I are responsible for are assisting with obtaining insurance, arranging transportation for discharge, and ensuring bed availability for the patient ’ s next facility. It is very important that we assess each individual within the hospital to hopefully have a positive impact on their life and reduce the chance for readmission to WellSpan Ephrata Community Hospital. Future Goals: My goals for the future include working towards becoming a licensed clinical social worker. I would like to work within a medical setting, specifically with the behavioral health population. Department of Social Work
