MSW Field Profiles 2021-22

MSW Carolyn Kornak 2021 Agency Name: Service Access and Management, Inc. Profile: Service Access and Management, Inc. helps individuals and families access resources across Pennsylvania and part of New Jersey. I have been placed in the Intellectual and Developmental Disabilities Department at SAM. This department helps connect people with Intellectual and Developmental Disabilities to community resources and supports. Field Placement Responsibilities: At SAM, I will be reaching out to consumers to evaluate their experiences with the agency's support coordination, and analyzing survey data. I will be learning about research into effective supports coordination and case management with individuals. I will also attend staff meetings and be able to shadow Licensed Social Workers in their case management roles. Future Goals: I hope to obtain my MSW degree, become licensed in my profession and help children and families through my work. Department of Social Work
