MSW Field Profiles 2020-21

MSW Danielle Ortiz 2020 Agency Name: Londonderry Village Profile: Londonderry Village is a continuing care retirement community in Palmyra, PA. Their mission is to provide quality housing, supportive services, and healthcare with love and servanthood. Londonderry Village offers a traditional nursing home unit that is mostly used for rehab patients and they also have 6 Greenhouses. The Greenhouses are a family setting with no more than 10 elders in a house. Field Placement Responsibilities: Some of the responsibilities that I have at Londonderry Village are giving Brief Interview for Mental Status assessments and Patient Health Questionnaire assessments to the elders in the facility. I am responsible for entering the assessments into MDS and understanding what a significant change in the assessments are and the steps to take when it happens. Other responsibilities are to select appropriate interventions with a focus on non - pharmacological inventions. As an intern I will also be developing individualized care, providing emotional support visits, and advocating for elders when needed. I will also be discharging elders back to the community with the services they need to stay safe. Future Goals: My future goals are to work with veterans who need assistance finding services and those who have a mental illness. Eventually, I would like to obtain my LCSW. Department of Social Work
