MSW Ashley Mest 2020 Agency Name: The Highlands at Wyomissing Profile: The Highlands at Wyomissing is a continuing care retirement community in Wyomissing. The Highlands has independent living, assisted living and skilled nursing. The Highlands has recently opened a short - term rehabilitation unit in the community. The goal of the short term rehab unit is to give clients the opportunity to heal and improve their physical wellbeing in order to get back to their typical lives. Field Placement Responsibilities: The MSW intern ’ s role at The Highlands at Wyomissing is to create policies, accept referrals and be knowledgeable about short term rehabilitation and all that comes with fast paced case management. During my placement, I will be making and accepting referrals from the hospital and other private referrals. I will do short term, fast paced case management when I follow clients throughout their stay on the unit. The MSW intern ’ s opinion is taken into consideration in order to create procedures that allow for smooth admissions, transitions and discharges. The MSW intern participates in daily and weekly meetings and care conferences In order to ensure that clients goals are being met and they are on track for a safe and successful discharge. Future Goals: I believe that I would like to do Medical Social Work in a hospital setting. I enjoy the fast - paced environment and getting to meet and help many people. Department of Social Work
