MSW Kayla Bedoya 2019 Agency Name: Good Shepherd Rehabilitation Profile: Good Shepherd Home at ConradW. Raker Center in Allentown offers nursing care for persons with physical disabilities whose primary diagnoses include cerebral palsy, muscular dystrophy, quadriplegia or Multiple Sclerosis. Good Shepherd focuses on motivating and empowering those they serve to achieve best outcomes and maximize recovery. Field Placement Responsibilities: At Good Shepherd, my field placement is within the Long Term Care Management team at ConradW. Raker Center. My responsibilities include maintaining a small caseload of residents experiencing a variety of physical disabilities needing long term care, including MS and cerebral palsy among others. The role of a social worker at Good Shepherd is to act as a resident advocate and to work on an interdisciplinary team. Along with managing a small caseload, I will be responsible for participating in teammeetings, contributing to care plans, and developing small groups for residents to participate in. Additionally, my responsibility will be to ensure that the resident's needs are being met. Future Goals: In the future, I'd like work in hospice or palliative care, spending time with a variety of clients. Being in Long Term Care Management has allowedme the opportunity to explore new roles in social work. Department of Social Work
