BSW Jose Manuel Bueno 2021 Agency Name: Empowerment Behavioral Health LLC Profile: Empowerment Behavioral Health provides services that are in - depth, specialized, and are high quality counseling services. The agency serves individuals and families within Berks County to assist them in overcoming their personal challenges, achieve their goals, and improve their insight and capacity for productivity for their lifetime. Field Placement Responsibilities: At Empowerment Behavioral Health, I am the school - based coordinator and handle the clients who receive counseling at the eight schools of the Wilson School District. In my position, I handle all enrollment paperwork - getting it printed, distributed, collected, and filed. I also answer all phone calls and questions regarding the enrollment process, perform accurate scheduling for appointments per the agency ’ s policy, and follow the process for checking in and checking out all patients to ensure accuracy. In addition, I also shadow client sessions and evaluations, score assessments, and dictate the psychiatrist ’ s evaluations. Future Goals: In the future, I would like to be a mental health social worker to have the ability to empower individuals with mental illness and their families, and careers to lead fulfilling, independent lives.
