BSW Lydia Mascia 2021 Agency Name: Diakon Profile: Diakon is a private, non - profit organization that provides intensive services that are designed to aid children and youths in out - of - home placements such as foster or residential care for whom the primary plan is reunification with their biological parent or parents. The program prepares family members for the reunification by providing intakes, assessments and trainings to address the families needs. Field Placement Responsibilities: My responsibilities at the agency are to work alongside case managers. I help the case managers with their intake and assessment forms as well as adding new information into the family binders. I attend home visits, supervised visits, zoommeetings and faculty meetings. In the meetings, I advocate for the clients and broker them to services that they may need. I will also get the opportunity to attend court cases. Future Goals: I plan to work towards getting my MSW and get a job in an adoption agency. Department of Social Work
