BSW Lindsey Smith 2020 Agency Name: Co - County Wellness Services. Profile: Co - County Wellness Services is an agency that provides free, preventative care to the community through service, education, and advocacy. This agency works to prevent and treat HIV/AIDS, other sexually transmitted diseases, and Teen Pregnancy. The agency provides case management, testing, treatment, and counseling to individuals in the community experiencing these challenges. Field Placement Responsibilities: The responsibilities of a contact tracer at Co - County Wellness Services include reaching out to individuals who have been in close contact with someone who has tested positive for COVID - 19. Once the individual is reached, the contact tracer requests that they quarantine and report any symptoms they may experience. This is made easier through software called Sara Alert, which allows those who are enrolled to be monitored daily for symptoms. It is the responsibility of the contact tracer to enroll contacts in this system. The contacts may also be monitored daily by the contact tracers themselves. It is the contact tracer ’ s responsibility to also keep accurate notes on interactions with contacts, broker necessary resources, and practice confidentiality throughout this process. Future Goals: Provide information and resources regarding sexual health education to youth. Many schools have a lack of comprehensive sex education.
