BSW Alliyah Yarbough 2019 Agency Name: Pennsylvania Mentor Profile: Pennsylvania Mentor is a home and community based human services agency providing services to individuals with intellectual or developmental disabilities, emotional, behavioral, and mental health issues. PA Mentor has a number of services working to enhance the lives of children and adults in the community. Field Placement Responsibilities: My field placement responsibilities include but are not limited to assisting my supervisor with daily tasks such as evaluations, assessments, school meetings, group and one on one meetings with clients. Additionally, I will broker clients to agencies that have resources that PA Mentor may not provide. Future Goals: My Future goal is to become a Medical Social Worker and assist individuals and families in a health care setting. I would also like to helph families that may experience emotional trauma. Department of Social Work
