BSW Field Profiles 2019-20

BSW Samantha Stoneback 2019 Agency Name: Heartland Hospice Profile: Heartland Hospice is a facility that provides end of life care for those with terminal illnesses and assists the family members or caregivers in the grieving process by providing a wide array of services including spiritual care, music therapy and at home visits. Field Placement Responsibilities: In my placement at Heartland Hospice, I go on home and facility visits for the patients, take volunteer notes, participate in weekly interdisciplinary meetings with the staff, fill out intake forms, and work together to create care plans for the assigned patients. Future Goals: When I graduate Kutztown with my BSW, my plan is to work towards getting my MSW. After I get my masters, I hope to work in St. Lukes Hospital as a medical or hospice social worker.
