BSW Field Profiles 2019-20

BSW Carlie Mills 2019 Agency Name: Tower Health at Home Profile: Tower Health at Home is an agency that tends to the older population of Berks County. They provide various programs to their consumers to ensure that basic living needs are met. Social workers and in-home aides visit consumers frequently to alleviate the stressors of everyday life. This could include housekeeping, connecting consumers to various services, and many other personal care needs. Field Placement Responsibilities: While at Tower Health at Home, my responsibilities include observing the many social workers in the Care Management Department as they make daily home visits to consumers. During these visits I will observe needs assessments, which cover the consumer's medical conditions, mental status, and financial situation. These visits are also done to ensure that the consumers' program services are being fulfilled. My responsibilities also include learning about the caregiver and grandparent programs, along with the programs provided to the consumers. This will give me the knowledge of which consumers are qualified for these various programs. Future Goals: My future goals include obtaining my masters degree and then securing a job within the field of gerontology, mental health, or disabilities. Department of Social Work
