BSW Field Profiles 2018-19

BSW Keri Miller 2018-2019 Agency Name: LVH - 17th St. Health Advocacy Program Profile: The Health Advocacy Program trains college students, such as myself, to be Health Advocates. These Health Advocates work with clients to assess and identify social needs. After completing and intake resources and/or referrals are given to the client to address these needs. Field Placement Responsibilities: At my internship it is my responsibility to receive referrals for clients. I complete intake and demographics forms with them. After the intake, needs are identified and I broker resources to the clients. I also perform follow-up calls to clients weekly to check if the resources are helping or if they have any additional needs. Future Goals: I plan to receive my MSW and work in a hospital setting as a medical social worker. Department of Social Work
